In the event that you experience or witness an incident that may be rooted in bias, you may choose to report it through this new online form. Anyone can use this form and it is recommended that you file as soon as possible after the incident.
What is bias?
A bias-related incident or bias incident is motivated, in whole or in part, by the offender’s bias against the actual or perceived age, ancestry, color, disability, gender, gender identity, gender expression, national origin, race, religion, religious practices, sexual orientation, marital status, veteran status, or military status of the targeted person or group, but does not rise to the level of a criminal offense. When filing a report:
- provide as much information as possible by filling out as many fields as possible and thoroughly describing the incident where prompted.
- use the third person and past tense when describing the incident (ex: instead of saying “I am being called a slur,” please write “John Doe was walking down the hall in Doty Hall when he was called a slur by Jane Doe.”).
- you may file this report anonymously, keeping in mind that appropriate follow up can be more effectively facilitated if you provide your contact information. Follow up may include further investigation and contact from campus resources to address your needs following the incident.
This form should not be used to report incidents of sexual and interpersonal violence or gender discrimination that fall under the scope of Title IX.
